Home By Choice
Home By Choice is a program that is funded by
managed care organizations to help nursing home residents return to the community.
In order to qualify for Home By Choice, a person must be living in a nursing home, have Medicaid benefits, and be enrolled in Molina, United Healthcare, or Wellpoint for STAR+PLUS.
Home by Choice relocation specialists help nursing home residents arrange housing (e.g., move in with family, get apartments of their own, or move to assisted living facilities), apply for transition grants to pay for things like security/utility deposits, essential furniture, and start-up groceries. They also set up other community services (like banking and transportation) that are not paid for by the Medicaid program.
