In-Home Support
NCTAAA’s Care Coordination program pays for temporary services that help older adults at risk of nursing home placement to remain in their homes. Services may include light housekeeping, personal care, respite, minor home repairs necessary to make the home more accessible, leasing of emergency response devices, medical equipment and supplies, and one-time help with utility bills.
To qualify for the Care Coordination program, a person must be at least 60 years old and meet at least four of the following criteria.
- Has low income, with no more than $1,995 per month for single people and $2,705 per month for married couples
- Needs help with at least three activities of daily living (e.g., getting out of bed or a chair, walking, getting dressed, taking a bath or shower, eating, grooming, and toileting)
- Has been in an emergency department, hospital, rehabilitation facility, and/or nursing facility within the last four weeks
- Has a severe health condition, with the need for assistance on a regular basis
- Has no help from family or friends
